Did I just miscarry?

I started birth control 3 weeks ago, I started it the day before my period was suppose to start so I haven't had my period. 4 days ago I started the placebo pills and been spotting. Last night I had real bad cramps my whole lower body hurt. Today I passed a big bag like thing, it was like 2 and a half inches long and a few cm at one end and an inch and a half at the other end. The long end was closed the short end had a hole in it. I know this because I rinsed it under water. The outside came clean but the inside was still kinda dark purplish red. I never had this happen to me before.......
