Introductions when you don't hear the other person's name clearly

Suppose you're meeting someone and shaking hands with them, but when they say their name, you don't hear it clearly or it's weird. Is it proper to continue anyway by saying your name and shaking hands and then ask them to repeat it, or do you ask sorry could you repeat your name for me, and only then introduce yourself? I like to say their name when I shake hands but I can't if I didn't hear it.

 

In other words, which is correct?

(A)

Me: Hi.

Them: Hi, I'm [ grxshimuphh].

Me: Hi, I'm mfan (shaking hands). Nice to meet you. Would you repeat your name for me?

 

or

(B)

Me: Hi

Them: Hi, I'm [ grxshimuphh].

Me: I'm sorry...?

Them: Greg Murphy.

Me: Hi Greg, I'm mfan, nice to meet you.

I would use this one when first meeting someone in a formal setting. If you are just meeting very informally then I think either one would be just fine.

I am not so good at remembering names, so I often ask people for their name multiple times (usually once each time we meet). Usually, they are flattered that I care enough to put an effort into it and don't mind helping me see my way through the "name brain fog".
