I did not completely understand which way you meant with down. When talking about a penis, up and down is relative. I'll talk about the way that seems most logic to me. If I got this wrong, say something.

So when you get an erection your foreskin starts to retract and your glans gets exposed. Well, retraction of the foreskin is important for two things: hygiene and sexual intercourse. This retraction during erection is important and natural. That's why I think you should not keep the skin down.

Now, it can be hurting on two places. Around all the foreskin because it is too stretched or just on the down side of the glans, on the frenum (that little amount of skin that sort of holds back the glans). Either way, it can be solved in a natural way or by surgery.

The natural way is to keep the foreskin retracted as long as possible preferably everyday. Yes it can hurt a bit, but you'll thank yourself for that. The foreskin is not very elastic, but with time it stretches a lot. It's important to never force back too much, if you do, the skin may tear and that would hurt even more.

But with some men, sometimes, the foreskin just won't stretch and so you should choose surgery. Which is not painful, just very annoying for one long week...

Hope you understood everything.
