I had a disability advocate fill it out last year and paid a nice price for it.. ouch . Please give good thoughts my son is accepted by the Canada Revenue Agency this time . Seraphim Posted June 7, 2022 Author Share Posted June 7, 2022 Yay! My doctor filled out and signed the form and won’t charge me. I make the 6 hour round trip to pick it up tomorrow. Then send it by registered mail to the CRA and wait . 1 Link to comment
