The whole teeth issue; it's harder to think about it then actually do it. And as for the swallowing issue; he's not really cumming "more than once" during his orgasm. He's actually shooting various ropes of ejaculate out in succession. When he orgasms, leave your mouth open over his penis while still lightly sucking, and then swallow after he lets his entire load out into your mouth.

That's what seems the easiest and it's what my girlfriend does.
